The Application Form has PDF fields to make it easier to complete the form. Once you have completed the form, please print and sign the appication.
The Candidate should provide with application:
- Copies of any documents showing attendance at TRIZ training
- Copies of any TRIZ related papers or projects
- Any letters of recommendation
- Two (2) forms of photo ID (Driver’s license, passport, etc) Proper ID will also be required when checking into the testing site.

An application for a TRIZ Practitioner Certification test should be submitted no later than four weeks prior to the test.
An application for a TRIZ Specialist Certification test should be submitted no later than two months prior to the test.
